Description
This appointment will require you to fill out a health history, include a brief discussion and assessment with your therapist followed by your massage. Total length of appointment, 60 minutes. Please be sure to arrive a few minutes early for this appointment if you did not complete the electronic version of your health history form. Direct billing available for most insurance companies. If you plan on direct billing, please bring your insurance benefit card with you along with a copy of your doctor's referral if required. Thank you for booking, we look forward to meeting you!
